Needs Assessment and Support Planning Policy – Family Wraparound Service (Ashkirk)
This policy sets out our approach to undertaking the assessment of clients’ needs and develop their support plans. It applies to all families residing in the Family Wraparound Service (Ashkirk).
This policy explains how Bernicia’s Family Wrap-Around Service assesses the needs of families living in supported accommodation and plans support to help them achieve their goals and live independently. The service uses a whole-family approach, looking at the needs, strengths, risks and goals of the entire family rather than focusing on individuals separately.
The policy aims to make support planning consistent, easy to understand, family-led and focused on positive outcomes. Families are involved in identifying their own goals, and support plans must be completed within two weeks of moving into the service. Risk factors are considered as part of the assessment process, and staff involved in support planning must receive appropriate training.
Overall, the policy is about ensuring families receive coordinated, personalised support that helps them build stability, strengthen support networks and sustain independent living.
